¡Para un Corazon Saludable! For a Healthy Heart
In its third year, ¡Para un Corazón Saludable! is the Alliance’s national initiative to raise awareness and understanding of heart health, including atherosclerosis, in Hispanic communities. The program established a landmark partnership with the American College of Cardiology (ACC) to found and guide the National Hispanic Cardiologists Leadership Network, the first health professional network of Hispanic cardiologists and those that serve Hispanic communities, to be the leading professional voice on Hispanic heart health. The Network efforts are being implemented in workforce development, patient education and support, international efforts, and science and public policy. In 2009, community heart health education and screening programs were a key aspect of ¡Vive tu Vida! Get Up! Get Moving!™ family healthy lifestyle events. In addition, all patients and providers reached by this initiative receive personalized assistance through the Alliance’s bilingual Su Familia Helpline. Hispanic Community Engagement: Hypertension
With the theme “Hypertension: Control is in Your Hands” this program is in its third year of implementation. The program currently focuses on 1) enhancing community capacity to implement a promotora/patient navigator/case management model program to ensure treatment compliance of Hispanics diagnosed with hypertension; 2) providing bilingual educational materials for consumers; 3) providing a training curriculum for health educators/promotoras; 4) providing information and referrals through the Su Familia Helpline; and 5) implementing a media component. The program is being implemented in partnership with San Ysidro Health Center in San Ysidro, California and is funded by Novartis Pharmaceutical.
